SUPPLEMENTAL DISPOSITION ENTRY
This Day, June 30, 2022 This matter came on for consideration. It is the order, judgment and decree of this court that This matter comes before the Court by way of Probation Officer Griffith. She informs the Court that the defendant, Sandra Clark, has been accepted into the Surest Path Recovery Program. She recommends defendant Clark be transported back to the Findlay Municipal Court on the CCNO morning bus on 7/01/22 at approximately 9:30am and released to probation staff. Probation staff will then release defendant Clark to her son who will ensure her arrival at the facility no later than 4:00pm. The recommendation is well taken. Therefore, it is the order of that Court that defendant Clark be released from CCNO on 7/01/22 to be transported on the morning bus to the Court and released to the custody of probation staff. Probation shall release defendant Clark to her son to be transported to the treatment facility. Successful completion of the treatment facility shall be an added condition of probation and the defendant must sign all necessary releases of information to ensure the facility is updating the Court on the defendant's progress in treatment as well as her departure, successful or unsuccessful, from the program. Defendant Clark shall continue to report to the probation department as previously ordered until her pending revocation hearing which shall be in approximately 60 days from today. This Day, April 22, 2022 This matter came on for consideration. It is the order, judgment and decree of this court that Defendant Sandra R Clark, whom is under the supervision of this Court's Probation Department was arrested Thursday April 21, 2022 by Probation Officers and taken into custody for violations of the terms and conditions of community control sanctions. A commitment after conviction issued for the Wood County Sheriff's Office jail, with whom the City of Findlay contracts for the incarceration of this Court's defendants. Defendant Clark was transported to the Wood County Jail for incarceration pending hearing, however was turned away from the Wood County Jail for medical reasons. Defendant Clark was transported back to the Court where she was placed under house arrest and ordered to report to the Court Monday morning April 25, 2022. The defendant was not fitted with a GPS unit due to a pending medical procedure which prohibited the utilization of the GPS ankle bracelet.
